Déficits immunitaires primitifs : à propos de 12 observations

RÉSUMÉ :

Introduction : Les déficits immunitaires primitifs (DIP) sont des maladies rares et sont caractérisés par des infections sévères et répétées. Objectif : Étude du profil clinique, thérapeutique et évolutif des DIP. Patients et méthodes : L’étude a porté sur 12 patients atteints d’un DIP confirmé par le bilan immunologique. Résultats : L’âge moyen de survenue des symptômes a été de 6 ans, le sex ratio (H/F) de 5 et le délai diagnostique moyen de 7 ans. Les symptômes ont été à type d’infections sévères, répétées dans tous les cas, le plus souvent respiratoire, compliquées d’une dilatation des bronches chez 8 patients (62 %). Il s’agissait d’une agamma ou d’une hypogammaglobulinémie dans 11 cas et d’une hyper IgM dans le dernier cas. Tous les patients ont été traités par antibiothérapie et veinoglobulines. La durée moyenne du suivi a été de 12 ans et l’évolution a été favorable dans tous les cas. Conclusion : Les DIP doivent être évoqués devant toute infection récidivante. Les veinoglobulines au long cours sont efficaces.

ABSTRACT: PRIMARY IMMUNODEFICIENCIES IN 12 OBSERVATIONS.
Introduction : Primary immunodeficiencies (PIDs) are rare and characterized by severe and repeated infections. Aim : To precise the clinical, therapeutic and evolutionary profile of PID. Patients and methods : the study concerned 12 patients with PID confirmed by the immunologic exploration. Results : Mean age of symptoms survenue was 6 years, sex ratio (H/F) of 5 and average duration of the disease before diagnostic of 7 years. Severe and repeated infections were present in all the cases, often respiratory with bronchus dilatation in 8 patients (62 %). It was an agamma or hypogammaglobulinemia in 11 cases and of hyper IgM in the last cases. All the patients underwent antibiotherapy and long course immunoglobulin substitution. Mean follow–up was of 12 years and evolution was favorable with a net reduction of infections in all the cases. Conclusion : PIDs must be call to mind in any repeated infection. Long course therapy with immunoglobulin is very efficiency.
